HDHP with HSA - lowest employee premium, high deductible, employee contributed HSA.
PPO Low Plan - lower deductible than the HDHP plan but copays before deductible for some services (such as Primary Care & Specialist visits).
PPO High Plan - highest employee premium, lowest deductible in-network, copays before deductible for some services.

All Plans:
- Cover the same type of eligible expenses
- Utilize the national BlueCross BlueShield network (BlueCard)
- Provide higher benefits when you use an in-network provider
- In-network preventive care covered at 100%
- Provide ultimate protection through an out-of-pocket maximum
- Do not require selection of a Primary Care Physician (although recommended)
- Do not require referrals for Specialist visits
