Are my dependents eligible?

Legally Married Spouses

Dependent children who meet the following guidelines are eligible to be covered up to age 26

Adult children shall include those who are a child of the plan participant, whether or not they are:

  • Married or not married;

  • Live at home;

  • A dependent on the employee’s tax return; or,

  • A student.

Eligible dependent children are eligible to be covered until their 26th birthdate except for the MEC plan which is covered until the end of the month.

When can I make changes to my benefits?

After initial eligibility as a new hire, you are able to make changes either at the annual *Open Enrollment period (typically held in November/December for January 1st) or if you have a qualified change in status (life event change).

Qualified changes in status include: marriage/divorce, birth or adoption of a child, change in child’s dependent status, death of a dependent, change in residence, or change in spouse’s benefits or employment status. If you experience a qualified change in status, please notify Human Resources within 30 days of the event.

*Please be aware that there is no Open Enrollment period applicable for Voluntary Life insurance. 

If you choose to enroll yourself, a spouse or child in NY Life Voluntary Life insurance after your initial eligibility as a new hire,  you may be required to go through medical underwriting and approval by New York Life for all Life insurance amounts you apply for unless certain circumstances apply. Application for enrollment outside of life events may only occur on a quarterly basis during the months of November, February, May & August. Dependents will not be allowed to enroll unless you also enroll and are approved by NY Life.

Enrollment Instructions

  1. Go to https://access.paylocity.com/

  2. Click on “Register New User”

  3. Click on “I don’t have a Registration Passcode”

  4. Enter [COMPANY CODE] as the Company ID (from the chart below)

  5. Enter your full Social Security Number and then re-enter your full Social Security Number

  6. Enter your Date of Birth and the click the orange Continue button

  7. The next page will ask you to create your username and password. These can be whatever you want them to be as long as they follow the requirements on the page

  8. The next page should ask you to create security questions. Choose from the options on this page and enter your answers.
